Loss Control Standards Summary

LCS #1 – Commitment to Safety/Risk Management

Requirement: Have a written statement of senior management's commitment to risk management and employee safety, approved by your governing body, which is communicated annually to all employees and posted in the workplace.

Measure of Compliance: Annually, submit form verifying continual compliance with requirements by the end of June.

LCS #2 – Responsibility for Loss Control

Requirement: Designate a management level person with the responsibility and authority to implement Loss Control programs.

Measure of Compliance: Annually, submit form verifying continual compliance with requirements by the end of June.

LCS #3 – Hazard Inspections

Requirement: 1. Monthly, conduct documented self-inspections at all high risk facilities. 2. Annually, conduct documented self-inspections at all facilities. 3. Track and document corrective actions to completion.

Best Practice: Monthly, conduct documented self-inspections at all facilities.

Measure of Compliance: Annually, submit form verifying continual compliance with requirements by the end of June.

LCS #4 – Incident Investigations

Requirement: 1. Have a written procedure for conducting claims investigations. 2. Within two weeks of notification by claimant, complete written incident investigations for every Workers’ Compensation and Liability claim filed with PennPRIME/Inservco, even zero dollar. 3. Track and document corrective actions to completion.

Best Practice: Complete written incident investigations for all incidents, whether or not a claim is filed.

Measure of Compliance: Annually, submit form verifying continual compliance with requirements by the end of June.

LCS #5 – Certificates of Insurance (COI)

Requirement: Adopt a written procedure, incorporating minimum PennPRIME criteria, requiring COIs be obtained from contractors. Maintain a list of contractors and current COIs for each.

Best Practice: Have all COIs reviewed annually by your insurance representative or PennPRIME and address findings.

Measure of Compliance: Annually, submit form verifying continual compliance with requirements by the end of June.

LCS #6 – Seat Belt Use

Requirement: Enforce a written policy requiring seat belt use for all occupants of entity vehicles or other vehicles used on entity business.

Measure of Compliance: Annually, submit form verifying continual compliance with requirements by the end of June.

LCS #7 – Motor Vehicle Record (MVR) Review

Requirement: Have a written procedure to review MVRs to verify a valid driver’s license, prior to employment and every three years thereafter, for anyone authorized to drive vehicles on entity business.

Best Practice: Have a written policy for evaluation of the MVR and past accident history with criteria to classify results as acceptable, questionable or unacceptable and appropriate action taken.

Measure of Compliance: Annually, submit form verifying continual compliance with requirements by the end of June.

Recommendation – Cell Phone Use While Driving

Best Practice: Enforce a written policy prohibiting personal or entity cell phone use while driving on entity business or when driving an entity vehicle.

Measure of Completion:  Submit policy to PennPRIME for review and recommendations.
